The underlying diseases are secondary to what regular eye exams can detect. When eye doctors look at your eyes, they will look for markers or signs for eye diseases like macular degeneration, glaucoma, and cataract. They look for signs of the development of these diseases because these greatly affect the ability of people to see especially once they are in the advanced age.  Most of these diseases occur without evident symptoms during the early onset of the disease.
Getting regular comprehensive eye exams at least every five years is very important to check the condition of your eyes. Make sure that you visit an eye doctor to assess your eyesight and give the necessary treatment if disorders are identified during the eye exam.
